Apixaban ook bij aspirinegebruik effectiever dan warfarine voor strokepreventie bij atriumfibrilleren

Apixaban vs. warfarin with concomitant aspirin in patients with atrial fibrillation: insights from the ARISTOTLE trial.

Achtergrond

Van orale anticoagulantia is aangetoond dat ze effectiever zijn om beroerte te voorkomen dan plaatjesremmende therapie met alleen aspirine [1,2] of aspirine plus clopidogrel [3], bij patiënten met atriumfibrilleren [AF]. Patiënten met AF hebben vaak ook coronair arterielijden of andere indicaties voor aspirine [4,5]. Er zijn hoge bloedingfrequenties geobserveerd bij de combinatie van aspirinebehandeling en vitamine K antagonisten (VKA)[6-12]. Ook bij patiënten die recent een acuut coronair syndroom hadden doorgemaakt en die al plaatjesremmende behandeling kregen, werden meer bloedingen gezien bij patiënten die de nieuwe orale anticoagulantia kregen, dan bij patiënten op placebo [13,14]. Op basis van deze observaties wordt aanbevolen om plaatjesremmende en antistollingsbehandeling tot maximaal 12 maanden te combineren bij patiënten met AF en een coronaire stent, en om daarna de antiplaatjestherapie te staken [15-17].
De ARISTOTLE studie toonde superieure effectiviteit aan van apixaban ten opzichte van warfarine voor preventie van beroerte in AF patiënten, en apixaban gaf minder ernstige bloedingen [18]. Deze analyse van de ARISTOTLE trial onderzocht de effectiviteit en veiligheid van apixaban in vergelijking tot warfarine bij patiënten die gelijktijdig een lage dosering aspirine kregen.


Belangrijkste resultaten

  • Vergelijkbare dalingen in het aantal beroertes werden gezien met apixaban in vergelijking tot warfarine in aspirinegebruikers (1.12% vs. 1.91%, HR: 0.58, 95%CI: 0.39-0.85) en niet-gebruikers (1.11% vs. 1.32%, HR: 0.84, 95%CI: 0.66-1.07, P voor behandel-interacti= 0.10). Er werden geen effecten van aspirinegebruik gezien op het effect van apixaban vs. warfarine op het aantal ischaemische beroertes, myocardinfarct of sterfte.
  • Apixaban resulteerde in consistente dalingen van bloedingen in vergelijking tot warfarine, in aspirinegebruikers (3.10% vs. 3.92% met warfarine, HR: 0.77, 95%CI: 0.60-0.99) en niet-gebruikers (1.82% vs. 2.78%, HR: 0.65, 95%CI: 0.55-0.78, P voor interactie: 0.29). Tevens werden consistente voordelen gezien met apixaban ten opzichte van warfarine bij aspirinegebruikers en niet-gebruikers voor hemorragische beroerte, ernstige of klinisch relevante niet-ernstige bloedingen en ieder type bloeding.
  • Vergelijkbare resultaten werden gezien in analyses die rekening hielden met of een patiënt aspirine nam op het moment van de bloeding.
  • Na correctie voor de neiging om aspirine te gebruiken voor verschillen in prognostisch belangrijke baseline variabelen geassocieerd met aspirinegebruik, hadden aspirinegebruikers vergelijkbare uitkomst rates in vergelijking tot niet-gebruikers. Alleen na verdere correctie voor post-randomisatievariabelen geassoacieerd met aspirinegebruik, was er een trend richting dat aspirinegebruikers meer beroertes of systemische embolie, ischaemische beroerte en myocardinfarct lieten zien.
 

Conclusie

Deze analyse van de ARISTOTLE trial toont aan dat het voordeel van apixaban ten opzichte van warfarine in het verminderen van beroerte of systemische embolie bij patiënten met AF niet wordt beïnvloed door gelijktijdig aspirinegebruik. Wanneer een sterke indicatie bestaat voor gelijktijdige behandeling met aspirine en orale antistolling, lijkt apixaban een veiligere keuze dan warfarine.
Aangezien aspirinegebruik in deze studie niet geblindeerd, noch gerandomiseerd was, moeten nieuwe gerandomiseerde klinische studies de optimale antitrombotische strategie en de duur ervan verder definiëren, in patiënten met zowel AF als een indicatie voor plaatjesremmende therapie.
