Inflammation impairs eNOS activation by HDL in patients with acute coronary syndrome.

Achtergrond

HDL transporteert cholesterol van perifere cellen naar de lever, voor excretie via gal en faeces. Dit reverse cholesterol transport (RCT) voorkomt arteriële cholesterolopstapeling [1]. HDL heeft ook een functie in het onderhoud van de vasculaire endotheelfunctie, door vasculaire tonus, inflammatie en endotheelcelhomeostase en integriteit te beïnvloeden [2]. In gekweekte endotheelcellen, verhoogt HDL de aanwezigheid van endotheel NO synthase (eNOS) [3-5] en stimuleert het eNOS activatie via diverse parallele signaalroutes.
Recent is aangetoond dat HDL van patiënten met acuut coronair syndroom (ACS) de endotheliale NO productie niet kan stimuleren [6]. Anti-oxidatieve capaciteiten zijn ook verstoord [7], hoewel cellulair cholesterolefflux nog wel werd gestimuleerd [6].
De inflammatoire respons in ACS wordt gekarakteriseerd door verhoogde afgifte van bepaalde eiwitten door de lever. Tijdens deze acute-fase response (APR), wordt HDL grotendeels veranderd qua structuur als qua samenstelling [8,9]. Dit kan de atheroprotectieve capaciteiten van HDL beïnvloeden.
45 ACS patiënten met ST-elevatie myocardinfarct (STEMI) werden bestudeerd, die een APR van verschillende intensiteit ontwikkelden, ondanks vergelijkbare ernst van het STEMI [10]. HDL avn deze patiënten werd gekarakteriseerd om in te schatten hoe APR de atheroprotectieve functie van HDL tijdens ACS beïnvloedt. Patiënten werden gestratificeerd in kwartielen op grond van deltaCRP (waarde bij opname werd afgetrokken van de hoogste waarde (APR piek) tussen opname en ontslag).

 
Belangrijkste resultaten

  • HDL cholesterol, apoA-I en apoA-II niveaus correleren significant en negatief met deltaCRP, terwijl LpA-I:A-II concentraties positief correleren met deltaCRP.
  • Inflammatie had een grote invloed op HDL structuur: een verandering richting grotere HDL deeltjes werd geobserveerd.
  • Een significante omgekeerde correlatie werd gezien tussen delta CRP en de capaciteit van HDL van patiënten om eNOS te activeren. De mogelijkheid van HDL om NO productie te stimuleren verschilde ook tussen patiëntkwartielen. HDL van patiënten in de eerste twee kwartielen liet een normale mogelijkheid om eNOS te activeren en om NO productie te stimuleren zien, terwijl HDL van patiënten in de 3e of 4e deltaCRP kwartielen een significant verlaagde capaciteit had om eNOS te activeren en NO productie te stimuleren.
  • De capaciteit om cellulaire cholesterolefflux te stimuleren was vergelijkbaar in alle geteste sera van alle patiënten. Een significante positieve correlatie werd echter gezien tussen deltaCRP en delta ABCG-1-gemedieerde cholesterolefflux, hetgeen consistent is met de verandering richting grotere HDL deeltjes.
 

Conclusie

Deze studie toont aan dat HDL van ACS patiënten die een belangrijke inflammatoire response ontwikkelen tijdens STEMI, een verstoorde capaciteit heeft om eNOS te stimuleren en dat het een hermodulering van de structuur ondergaat. Slechts een subgroep patiënten die een STEMI doormaakt heeft dus circulerend HDL dat tekortschiet in het stimuleren van endotheliaal eNOS en NO productie. Wanneer APR beperkt is, is de HDL structuur en functie behouden tijdens STEMI. Cellulaire cholesterolefflux via diverse signaalroutes was behouden in ACS patiënten, ongeacht of een inflammatoire reactie werd gezien.
